In this section you will be able to download the patient registrations forms, that should be filled out before your first appointment. We normally mail a copy of these forms to you prior to your first visit, but if time does not permit for this, we would like you to consider this option. This will save you and our office time on the day of your first appointment.
The file was created using Microsoft Word, but is saved in a rich text format and should be accessible from most word processing programs. After downloading the file, you can either print the forms and fill them out, or you can type directly on the forms using your word processor and then print. For your convenience, you can now e-mail your completed registration forms to our office. To do this, download the form, using your word processor type in your responses, then save the file using the following convention (registration_yourname.doc). Once the file is saved, return to this page and click here to send an e-mail to us at hdh@charterinternet.com. Include the file you created as an attachment to your e-mail.
